Program Description
Medication safety is paramount in preventing errors and safeguarding lives, and pharmacists play a crucial role in this process. By meticulously reviewing prescriptions, ensuring correct dosages, and monitoring for potential drug interactions, pharmacists act as a vital line of defense against medication-related mishaps. Their expertise not only helps in identifying and mitigating risks but also in educating patients about proper medication use and adherence. Through these vigilant practices, pharmacists contribute significantly to reducing the incidence of medication errors, ultimately saving lives and enhancing the quality of patient care, one dose at a time.
At the end of this presentation, pharmacists and pharmacy technicians should be able to:
- Describe the difference between a medication error and a close call or near miss
- Evaluate inpatient and outpatient medication errors
- List methods to prevent medication errors
