Why Patients Don’t Always Follow Directions: Lessons Learned from Health Coaches Working with Pharmacists. Program 183
Program Description
This is the third virtual North Suburban Pharmacists of Chicagoland continuing pharmacy education program. NSP is committed to providing thoughtful and engaging programs. Your support of ALL pharmacy organizations (ex. IPhA, NSP, APhA, etc.) is more important now than ever. How we are practicing our profession during this difficult time has been challenging and ever evolving. We will be working to provide virtual programs until it is deemed safe to provide live programs again. I am hopeful that you will participate and grow your knowledge with us.
We have seen during the Covid-19 pandemic that different groups of the population have been affected disproportionately. We will discuss the who, what , where, and why this is happening based on studies related to diabetes patients.
Pharmacist Program Objectives
At the conclusion of the program, the pharmacist and pharmacy technician will be able to:
1. Define the social determinants of health
2. Explain the impact of social determinants of health on a person’s ability to self- manage of type 2 diabetes
3. Distinguish intentional versus unintentional poor adherence
4. Consider new approaches to assessing adherence barriers

This activity has been designed to meet the educational needs of pharmacists and pharmacy technicians.
